From coordinating services and assisting with hospital discharge planning to connecting families with valuable resources, case managers play an essential role in helping patients receive the care and support they need.

What Does a Case Manager Do?

Case managers serve as an important connection between patients, families, healthcare providers, and community resources. While their responsibilities vary depending on where they work, their primary goal is to help coordinate appropriate care while supporting the needs of each individual patient.
